The Queensland Community Vitality Initiative participant pathway combines personal reflection, shared learning and realistic follow-through. It is designed to help people turn good information into actions that feel possible in everyday life.

Join a welcoming 2.5-hour workshop with people from your community.
Choose small, realistic actions that fit your life, needs and priorities.
Use practical resources and continued learning through Vital Health Roots.
Receive six months of follow-up prompts, encouragement and community connection.
Keep building on what helps, in a way that remains useful and sustainable.
Step two
The Community Vitality Profile is planned as a guided self-reflection tool. It will help participants notice strengths, priorities and areas they may want to explore.
It is not a medical test, diagnostic assessment or substitute for advice from a qualified health professional. Participants will choose what they wish to reflect on and what actions, if any, they want to take.

Participants would be able to revisit key ideas through Vital Health Roots and receive a paced series of useful prompts, resources and check-ins.
The aim is continuity, not surveillance or pressure. Participants remain in control of what they use, what they share and how they continue.
Evaluation will focus on feasibility, accessibility, participant experience and useful indicators of personal and community vitality.
